Our
Surgeons
Each
or our Surgeons is a Board
Certified Diplomate of the American Board of Surgery and is
a Fellow of the American
College of Surgeons. This means they have achieved the highest
standards of excellence in their field. Each has acquired specialized
knowledge and experience related to the diagnosis, preoperative,
operative and postoperative management, including the management
of complications, in nine primary components of surgery. The surgeons
are also well trained in numerous other areas of medicine.
Our
surgeons take great pride in their ability to apply cutting
edge technology with time proven techniques, in providing
the best possible surgical care to their patients. They consistently
exceed the biannual state requirements for continuing medical
education. They remain active in academic appointments and
are Preceptors with the Florida Hospital Family Practice Residency
program.
Lastly,
our surgeons find great reward in their work which is evidenced
by the friendly, thorough service to their patients and high
quality of their work.

Appointments
Appointments may be made by calling our office at 407-647-1331, Monday thru Thursday from 8:30 A.M. to 5:00 P.M., and Friday 8:30 A.M to 2:00 P.M.
We
make every effort to see patients on-time. However, emergencies
do arise from time to time and we ask for your understanding.
If
you have a problem and need to be worked into the schedule,
please call us as early as possible. You will be scheduled
with your surgeon, when available, or with the first available
member of our group.
If
you are unable to keep a scheduled appointment, please let us know
right away so that we may give that time to another patient.
On
your first visit to the office, you will be asked to complete
some forms and we request that you come fifteen (15) minutes
ahead of your scheduled time to do so. Please be certain that
you bring the following items with you:
- The
Patient Information

All blanks and questions need to be completed.

Your insurance card and plan booklets showing participating
doctors and facilities.
- All
x-rays, reports and medical records related to your visit.

Authorization from you Primary Care Physician (PCP), if
necessary (HMO or Managed Care only). Your visit may be
rescheduled if not pre-authorized.
If
you are having surgery in the office, we ask that you review
the list of blood thinning agents.

Fees
and Insurance
Please be prepared to pay for office visits when you are seen. If you have insurance, we will be happy to file a claim on your behalf and you need only pay any deductibles or co-payments not covered by your insurance. We accept cash, personal checks, Visa, MasterCard, and Discover.
Surgical
Associates is a member of many Preferred Provider Organizations
(PPO) and Health Maintenance Organizations (HMO). As participating
providers, we accept their payments in full, billing you only
for deductibles, co-insurance, co-payments and non-covered
items. If we are not members of your insurance plan, please
contact our office in advance of any surgery to discuss your
insurance benefits and payment arrangements.
If
your plan requires authorization for the office visit or procedures
done in the office, this must be completed when you arrive for the
appointment. Your visit or procedure may be canceled if the prior
approval is not completed. Please check with your insurance plan
or your Primary Care Provider (PCP) for help with this.
Please
do not hesitate to contact our office regarding our charges, assistance
with your insurance, payment arrangements or any other concerns
you may have.

If
Surgery is Recommended
Our
surgery scheduling department will work closely with you to make
sure all your questions are answered. Please inform us if you have
a hospital preference or any special needs or requests.

If
you need to be admitted to the hospital, your surgeon or one of
his partners will see you every day. One of the members of the group
is always on-call to provide 24 hour coverage for our patients.
Should
you require surgery, our office staff will pre-certify the procedure
with your insurance company. This is not a guarantee of benefits,
but a medical approval from your plan to proceed with the surgery.
We will make every attempt to use physicians and facilities participating
in your plan, but cannot guarantee the status of other providers
with your insurance. You should consult your plan materials regarding
participation of the Anesthesiologist, Pathologist, Radiologist,
etc. Our staff will be happy to explain this further and assist
you with these issues.
Please
review the list of blood thinning
agents prior to your surgery and follow the instructions
indicated there.

Final Word
The
surgeons and staff of Surgical Associates share a common goal to
give you our best. We encourage you to ask questions about your
care and treatment. We feel it is important for you to understand
what is being done for you and why.
